Being at Home Is Better

The Kaiser Foundation commissioned a study on the effects of hospitalizations on the elderly. The study found that even though the elderly were the fastest growing segment of hospital populations, the unique needs of those older patients were not a priority for those hospitals.

Interrupted sleep, unfamiliar surroundings and unappetizing food contributed to physical and mental deterioration even in cases where the original injury and/or illness were successfully treated. Furthermore, 1/3 of patients over the age of 70 left the hospital more disabled then when they arrived.

More and more research clearly show that, whenever possible, a home recovery lowers the risk of additional complications, including mental stress. The chances for a successful recovery at home increase significantly when there is a caregiver at home.

Reliable Home Health Corporation has worked with major HMO's in reducing unnecessary hospital admissions since 2012. Our Chronic Care Program has consistently reduced year over year all-cause admissions by more than 60%. We take patient centered approach to health care and work closely with the patient's PCP and/or specialists to create effective care plans. Our Care Managers will work with our field staff to help ensure wellness goals are being met.

Reliable's Care Transitions program has produced significant reductions in 30 day and 60 day re-admissions working with major health plans. Our unique approach empowers the patient to participate in his or her recovery and wellness. We help ensure that patients have what they need to recover safely at home.
